Our Services

We are also happy to work with other primary care providers to fill the gap for when their patients are homebound and unable to get to a physical clinic. So we will manage their care until they are able to return to their primary PCP. We can even follow our patients in many area skilled nursing facilities and assisted living facilities!  Our focus is to reduce unnecessary and avoidable hospitalizations.
